R.66 Documents to be submitted along with an application under sub-section (1) of section 59 before the Commissioner-
Every application referred to in sub-section (1) of section 59 shall be addressed to Commissioner and shall be presented in the office of the Commissioner alongwith following documents:
(i) Document in support of the fact that no proceedings are pending before any authority or court under the Act or for the purposes of the Act
(ii) Document in support of fee required to be paid for entertainment of application under Section 59.
(iii) Document in support of the contention raised by the applicant in his favour.
(iv) Copies of the judicial decision relied upon by the applicant.
(v) Copies of expert’s report, if any, relied upon by the applicant.
(vi) Any other document that the applicant may like to submit.
